Consent to collection and use of unique identification information

Need Korea LLC (“Need” or “We”) collects and uses your personal information as follows.

• Particulars of information to be collected: 

• Unique Identification Information Set Forth in the Copy of the ID card Submitted by You. We may collect information in relation to your Driver’s license number, passport number, and foreigner registration number

• The purpose of collection and use of information:

• To collect medical records in accordance with the procedures set forth in applicable medical laws and regulations.

• The period of retaining and using information:

• Until the earlier of (i) user’s withdrawal from service or (ii) expiration of the period of delegation for issuance of medical records; provided, however, that certain information may  be retained for a longer period as required under applicable laws and regulations

You may refuse to consent to the mandatory collection and use of unique identification information, but if you refuse, please note that your use of the Need services may be restricted.
