Insurance-Related Information. We may collect contact information, policy information, credit information, claims submitted, risk assessments, and other Personal Information insurance providers have used in their decision to provide you a policy or provide coverage under your policy.
Health Examination Appointment Information. We may collect information in relation to reservation of health examination appointments.
